Se realiza un despegamiento mucoperióstico de espesor completo, con un despegador o periostotomo. En primer lugar se despega el ángulo mesial del colgajo vestibular. Si es necesario se puede hacer un refuerzo del nervio bucal largo en el fondo de vestíbulo. Posteriormente se va despegando la fibromucosa del trígono retromolar. Es importante la continua irrigacion y aspiración para una adecuada visualización del campo operatorio.
